| All tax files must have ".IRS" as the extension. |

| If multiple forms are in 1 file, use your credit union's UCI code and credit union number. Example: UCI123.IRS |

| Important: U5498-FMV is a different form from the 5498, so please use the appropriate naming convention and keep it in it's own file, separate from the other forms. |
