2007 Tax Form Processing Order Acknowledgement

 Step One: Postage Prepayment
Postage Prepayment is required and must be received by December 14, 2007. A service charge of $0.04 per item with a maximum charge of $400.00, will apply if Personix has to advance the postage.
The correct way to calculate estimated postage is to take the number of forms you ordered and multiply that amount by $0.343. The small overpayment or underpayment will then be calculated when your invoice is prepared. Correctly estimating the postage will eliminate overpayments.

 Step Two: Data Submission
 Must be uploaded by January 11, 2007
    File Format: 2007 IRS
 File Compression Method Supported: PKZIP
 Data Submittal Options:
     1. Transmission*
   The preferred method of receiving your data is by directly transmitting through our website. To receive a login and password, please call Rebecca Navarrete at 800-872-7060 x211.
   2. 4mm tape. DDS1, DDS2, DDS3 or DDS4 formats. Please use new tapes.
   3. 3.5" Floppy disk
 
 If you choose any method other than transmission, please observe the following:
  1. You must clearly label all media with the credit union name, UCI, filename and quantity of forms.
  2. You must fill out an instruction form. This form will be used as a data verification tool, so processing will not commence until Personix has received it. Click here to view/print the form (Requires Adobe Acrobat reader. See link below to download the reader.).
  3. Please ship the media to:

 Step Three: Sign-Off
Once Personix has received your data, 3 random samples of each form will be created and faxed to you. PDI WILL NOT PRINT AND MAIL YOUR FORMS UNTIL WRITTEN SIGN-OFF HAS BEEN RECEIVED. Once the sign-off has been received your forms will mail by January 31st if data was received by the 11th of January. All others will be processed on a first in/first out basis.
 
MINIMUM CHARGE FOR YEAR-END TAX FORM PROCESSING IS: $100.00
